I applied online. The process took 3 months. I interviewed at Blueprint Test Prep (Philadelphia, PA) in Jan 2016
Interview
Paper application, followed by a brief phone interview, and then an invitation to make a teaching video. If there are any steps after that, I don't know yet. The interviewer was very friendly. The video involves introducing yourself and teaching one of two logical reasoning questions that they give you. The only downside is the waiting time to hear back-- when there's a gap between tests and no upcoming training/classes, it can be a long wait.
