Lack of communication betwen employers and managers. Turf battles. My own experience is that the Project managers have created turfs they want to defend and are willing to do whatever it takes to do that. Projects tend to suffer asa result because the information flow is restricted and the project is not handled properly. Project Managers tend to shift blame a lot more than is necessary because they want to look good to management and are willing to throw engineers under the bus if it gets them noticed.
