No safety focus, reactive approach. In my experience the Safety Manager is the most unprofessional and inexperience person I ever came across, with very little understanding in investigation methodology and operational risks. Their solution is to sack the workers involved in the incident rather than identifying the root causes and improve the organisation failings.
