Understaffed, leadership out of alignment (perhaps purposefully working against each other), fiefdoms, information hoarding, unwilling to off-board c-players, inability/unwillingness to learn from past mistakes -- more time spent on "messaging" how mistakes were in fact "positive". Overall, two cultures working against each other to create an incredibly frustrating and dysfunctional work environment.
