Management is rarely available to make decisions and gets angered when forced to make decisions. Employees that have the expertise to make decisions on management's behalf are not trusted and allowed to. Problems that come from poor planning from management get looked at as poor performance from employees rather than management issues. The number 1 priority is how something looks, with no consideration to how it works or how it would be implemented, and it's up to the implementers to make magic with little tools or planning.
