Too many toxic ideas are allowed to flourish within the company and maintain a strong foothold which sets the company back. There is extreme favoritism when it comes to promotions and a lot shady backdoor deals when it comes to moving people in a position. The departments have almost no communication with each other and like to point the finger at the other department for issues. Micromanagement is a huge problem at the company which doesn't allow departments to properly assess their own issues. Also nepotism is prevalent in the company even though the "rules" state against such practice, these rules aren't adhered to
