- Complete and fatal mismanagement of the company. There was a conflation of 'profit' with 'strategy' resulting in short-sighted decision making that failed to meaningfully develop the firm's capabilities. No real understanding of what the firm was actually offering beyond meaningless buzzwords. No real business acumen at the top of the organisation and a complete unwillingness to critically assess the company's performance resulting in the same mistakes being made. - Business development was a joke based on nothing more than personal contacts. No consideration given to what the company was actually offering, only how to win the work. Business development revolved around winning the work no matter what relationships were ruined along the way or whether the company could actually deliver the work to a high enough standard. - Toxic military culture within management that permeated through all aspects of the company. Junior employees treated simply as 'boots on the ground' with no consideration of their skills or career aspirations. Responsibility was pushed down by senior management onto middle managers with no proper guidance or support. When things went wrong no one at the top was held accountable.
