Reflections from the Road with Co-Founder & President Mat Kendall: Last month I took a trip to Indiana during which I spoke with practices that aren’t just surviving — they are growing, innovating and outperforming big systems on critical quality measures like blood pressure (BP) control. At Muncie Primary Care, I had the chance to catch up with Megan Zelasko, MD. It was incredible to see the clinic her father built in 1980 being transformed into a modern, high-performing hub for the community. With a team of 8 nurses dedicated to care management, they are already leaning into the future by using remote BP cuffs and scales to support patients right in their homes. A few things really stood out to me from these conversations: • The desire for better data: Even in a state with a strong Health Information Exchange (HIE), providers are lacking information that is simple and actionable. They’re looking to Aledade, Inc.'s technology to bridge those gaps. • Patient education as a differentiator: High-performing practices want their patients to know why they focus on their care needs - because it leads to better outcomes and a more personalized experience. • Deep local roots: When Dr. Zelasko renovated her clinic, she hired local employees who were current or former patients, showing real support for her entire community. It’s a privilege to partner with providers who are so "all-in" on their communities.
The Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) model encourages primary care organizations to utilize technology-enabled solutions to deliver personalized, preventive care to patients with chronic conditions. Jacob Shiff, chief AI and technology officer at the Centers for Medicare & Medicaid Services (CMS) Innovation Center, joins Joshua Israel and Sean Cavanaugh on The ACO Show to discuss how these advancements can improve patient outcomes and provide clinicians with more support.
Our time together in Atlanta reinforced that gathering in person is essential to align with our mission and celebrate the patients and primary care organizations we serve. This year's All Staff Retreat also celebrated our peer-nominated Core Values Awards winners and our various Employee Resource Groups, while also allowing us to give back through a service project for the American Diabetes Association. Health Affairs published a new article co-authored by Sean Cavanaugh, chief policy officer, Travis Broome, SVP of policy & economics, and Farzad Mostashari, MD, co-founder and CEO. The piece makes the case for policies that will continue to incentivize and advance value-based care, building on the success of the Medicare Shared Savings Program (MSSP), which has already saved Medicare billions. Through strategic policy adjustments, the Centers for Medicare & Medicaid Services (CMS) can ensure accountable care organizations (ACOs) remain a viable, long-term framework to improve patient outcomes and lower costs to the health care system.
Aledade has appointed Oraida Roman as Chief Commercial Officer to lead our health plan partnerships strategy. In this role, Roman will oversee more than 200 value-based contracts, focusing on creating strong alignment among health plans and Aledade’s primary care partners to deliver better health care and improve affordability.
