Lack of management accountability. Different departments push work and blame to other departments to avoid criticism and feedback.
Departments are treated differently in terms of importance and rewards i.e. sales and operation departments will be let out early from work and others won't.
Outside of sales, there is very little room to move upward in the company. Management positions are often given to people outside of the company instead of promoting within. This leads to a couple of months' learning curve for new managers which leads to decreased productivity and poor communication.
