People get blocked from being promoted constantly
Too much work being prioritized with constant context switching and constant production incidents. There's no-one caring about testing, there's not even proper test environments for the new items being developed!!! Not enough testers and no time to write automation and then production incidents happen.
Product team keep changing their mind on what to make
Too many people that have been there 10 years + that cause issues and feel like they are always right because they've been there for so long
