1. Mostly everyone is trying to mark their own territory and not ready to acknowledge what can be done to be improved 2. Blame blame blame!! Either blame the people or the process fully ignoring the context or understanding what happened 3. Everyone can do everything and in any amount of time! There are so many projects or focus areas running in parallel that you lose focus as to what’s the most important for the company 4. Micro management from the very top, inculcating a culture of fear which is subtle and you can feel how so called managers behave to the management’s email And I can go on and on.....
