Interview
I was referred by a recruiter and the interview process was fairly standard. It was comprised of the typical interview questions and two assignments: create a PowerPoint of an implementation process and then following the interview / presentation, compose meeting minutes.
Following the interview, I sent the minutes, as well as Thank You emails.
The feedback was that I didn't send the meeting minutes - it appeared that those weren't passed on by the internal HR department, but the "Thank you's" were. Based on the feedback, the hiring manager thought my "Thank You's" were the actual minutes. It was really odd and disappointing.

Interview
Standard interview. Two people in the panel, first a brief introduction of the company then they start to dig into the CV, asking example of previous works done and scenario of potential issue, that I am quite confident is what they are facing right now.
The interview went really well and they had me confirmed already for the second and final stage, as my recruiter confirmed as well a couple of days later. Then, all of a sudden, their Operation Manager I think made a complete U turn, blabbering something about 'not senior enough' (for the record I have more than 10 years of experience).
I would have rated the experience positive considering the people were friendly and the interview has been overall a positive experience, if it wasn't for the fact that the company seems to be growing pretty much like every other one related to Healthcare and the NHS (politic, internal pushes, power struggles et similia).
