Poor communication from CEO and other c-suite members, who often make drastic decisions about company direction, hiring/firing, etc. with little to no warning; little room for growth (career paths are haphazard at best and more often stagnant, barebones raises); culture of overwork

Poor communication. I was let go without a performance review, no heads up. Also, as a director, my hands were tied when trying to innovate.
