The only time I did not pass a phone interview was with Amazon. The guy asked to tell a story how I solved some problem. After listening pretty long and comprehensive story, he told that he actually wanted to listen about a problem that I personally created but not someone else. Just 5 min before the interview end he started asking technical questions. Then 2 min later he told that we run out of time, although the question was very simple but required a little more than 2 minutes to write a complete code and explain it. So the outcome was pretty obvious at that moment. I think it's good when software developers conduct interviews for other developers, but not all software developers are capable to conduct them in a proper way.
